Summer Program

“L.I.F.E is happy to announce another year of Summer fun! Our 2017 Summer Program will begin Monday, June 5th-Thursday, July 27th. This is a Day Treatment and Training program provided through DDD funding. Please discuss your options of receiving DTS services with your support coordinator.
Our summer program offers 8 weeks of fun exploring many different topics each week. There is a theme for each such as Animal Kingdom, Space etc. All the activities that week are centered on the theme, and typically there will be social games, crafting and special events to support that week’s theme.

Our program is broken down into several age groups. The younger age groups will be onsite and separated into groups based on age (4-7 years old). For the older groups, we will have community outings in addition to being on site with swimming opportunities. It will be important for you to look over the curriculum for the summer to see if the outings are appropriate for your child. The curriculum will be available in March for all groups.

You can enroll your child in a morning session or afternoon session based on availability.

The morning session will run from 8:30 a.m. to 11:30 a.m.

The afternoon session will run from 12:30 p.m. to 3:30 p.m.

You can enroll your child in a 2-day program Tuesdays and Thursdays, or Mondays and Wednesdays; or a 4-day program which runs Monday through Thursday, based on availability.

If your child requires a 1:1 staff member for them to be successful and enjoy the program, I would urge you to confirm this with your support coordinator. If your support coordinator approves an enhanced staffing ratio we will assign your child a 1:1 provider. If your child does not require 1:1 support, then they will be placed in a group.

The first step is to fill out the summer inquiry form. We will start accepting names and creating our waitlist once received. Please send the attachment to Sara Kleinman, sara.kleinman@laurensaz.org. She will be your contact regarding DTS questions or information.

For all new families starting in our DTS program, we will schedule an on-site visit where you can turn in the intake paperwork and see the campus.

Here is a guideline of how to receive services;

  1. Contact Support Coordinator and request evaluation of DTS (Day Treatment Summer) Services.
  2. The Support Coordinator will inform you of approval or denial. If approved, the Support Coordinator will create a Change in ISP for L.I.F.E. to review and sign.
  3. This will be resubmitted back to your Support Coordinator.
  4. Hours are approved and sent to L.I.F.E.

In an effort to collect all appropriate DDD requirements, if your child is placed within our DTS program we will need the following within two weeks of acceptance;

  1. Change of ISP
  2. ISP
  3. Completed DDD Intake

Once completed, we will need your DTS hours sent to Lauren’s Institute for Education by no later than two weeks prior to the start of the Summer Program.”
